At MediZen Revitalize L.L.C., we provide telehealth evaluation and treatment for a variety of common acute illnesses and minor medical concerns.
Acute Care Telehealth allows patients to connect with a licensed healthcare provider from the comfort of home, helping reduce unnecessary travel, wait times, and missed work or school when appropriate.
Treatment recommendations are based on clinical evaluation, medical history, symptoms, available information, and provider judgment.

Not all conditions can be safely evaluated or treated through telehealth.
If additional testing, imaging, procedures, or physical examination are needed, patients may be referred for in-person evaluation.

Includes provider evaluation, treatment recommendations, prescriptions when clinically appropriate, and follow-up guidance.
Additional follow-up visits may be recommended based on your condition and treatment plan.
Important Information
• Telehealth services are available only to patients physically located in Kansas at the time of the visit.
• Not all medical concerns can be managed virtually.
• Certain conditions may require in-person evaluation, urgent care evaluation, emergency department evaluation, laboratory testing, imaging, or specialist referral.
• Prescriptions are issued only when clinically appropriate.
• Medication requests do not guarantee a prescription.
• Individual treatment recommendations vary based on patient-specific factors.
Chronic Disease Management & Service Limitations
Acute Care Telehealth is intended for evaluation and treatment of common acute, non-emergency medical concerns.
This service is not a substitute for primary care.
Ongoing management of chronic medical conditions such as diabetes, hypertension, heart disease, COPD, asthma, thyroid disorders, chronic pain conditions, and other long-term health concerns should be coordinated through a primary care provider or appropriate specialist.
Patients requiring ongoing chronic disease management may be referred for continued care with their primary care provider or another appropriate healthcare professional.
At this time, MediZen Revitalize does not provide in-office procedures, intravenous (IV) therapy, imaging services, stat laboratory testing, fracture management, laceration repair, foreign body removal, or other services requiring an in-person physical examination or procedural intervention.
Certain conditions may require referral to an urgent care clinic, emergency department, primary care provider, or specialist for further evaluation and treatment.
