At MediZen Revitalize, we’ve designed a simple, medically supervised process to ensure your treatment is safe, personalized, and effective.

Step 1: Book Your Medical Consultation

Schedule your telehealth appointment with our licensed medical provider. During this consultation, we will review your medical history, weight loss goals, current medications, and overall health to determine whether treatment is appropriate.

Consultation fee: $49​​​

Step 2: Complete Your Secure Intake Form

​

After booking your appointment, you will complete a confidential patient intake form. This form provides important medical and health information that allows your provider to safely evaluate and personalize your treatment plan.

Your information is HIPAA compliant and securely protected.​​

Step 3: Meet With Your Provider and Begin Treatment

​

During your consultation, your provider will determine whether medication is safe and appropriate for you. If prescribed, treatment options and pricing will be reviewed.

Once approved and payment is completed, medication is prescribed and shipped directly to your home. Follow-up is required for continued treatment and shipment authorization.
