At MediZen Revitalize L.L.C., we are committed to delivering high-quality, patient-centered virtual medical care. Due to the nature of telehealth services and custom medical treatment, the following policies apply to all purchases and services.

1. Refund Policy

All payments made to MediZen Revitalize L.L.C. are final and non-refundable except under the limited circumstances described below.

Eligible Refund Circumstances

Refunds may be issued only in the following situations:

Duplicate Payments
If you are accidentally charged more than once for the same service, a refund will be issued for the duplicate transaction.

Service Not Rendered
If payment was made and no service was provided or scheduled, a written refund request may be submitted for review.

Medically Ineligible Prior to Treatment Initiation
If a provider determines you are medically ineligible for treatment before services have been initiated and before any prescription has been issued or submitted to a pharmacy, a partial refund may be granted. Evaluation and administrative fees will be deducted.

 
2. No Refund After Prescription Processing

Once any of the following occurs, refunds will not be granted:

  • A prescription has been written

  • A prescription has been transmitted to a pharmacy

  • A compounded medication has entered processing

  • A medication has been dispensed or shipped

Custom-compounded medications are prepared specifically for the individual patient and are not eligible for return or refund under federal and pharmacy regulations.

3. Cancellation & Rescheduling Policy

Appointments must be canceled or rescheduled at least 24 hours in advance.

  • Late cancellations and no-shows are subject to the full appointment fee

  • Fees for missed appointments are non-refundable

  • Patients may reschedule once per appointment with proper notice

Repeated missed appointments may result in dismissal from care at provider discretion.

4. Telehealth Consultations & Digital Deliverables

All telehealth consultations, medical evaluations, care plans, electronic documentation, and educational materials are non-refundable once delivered or accessed.

Payment secures provider time, clinical review, and medical decision-making, regardless of whether a prescription is issued.

5. Pharmacy Charges

MediZen Revitalize L.L.C. is not responsible for refunds, reshipments, delays, or replacement costs associated with third-party pharmacies.

Once a prescription has been transmitted to a pharmacy, all medication-related matters must be addressed directly with the dispensing pharmacy according to their policies.

Medication pricing, shipping timelines, and compounding schedules are controlled by the pharmacy, not MediZen Revitalize L.L.C.

 
6. Chargebacks & Payment Disputes

 

Initiating a chargeback for services rendered constitutes a violation of this agreement.

If a payment dispute is filed after services have been provided or prescriptions processed, MediZen Revitalize L.L.C. reserves the right to:

  • Provide documentation to the payment processor

  • Suspend or terminate services

  • Refer unpaid balances to collections where permitted by law

We strongly encourage patients to contact us directly before initiating any dispute.

7. Refund Request Procedure

If you believe a refund qualifies under this policy, please submit a written request to:

Include:

  • Full legal name

  • Date of service

  • Detailed reason for request

  • Proof of payment

Requests will be reviewed within 3–5 business days. Approval of any refund remains at the sole discretion of MediZen Revitalize L.L.C. in accordance with this policy.

8. Clinical & Legal Disclaimer

All services provided by MediZen Revitalize L.L.C. are based on clinical appropriateness and licensed medical provider judgment.

Purchasing a consultation or service does not guarantee:

  • A specific diagnosis

  • Approval of a requested medication

  • Issuance of a prescription

  • Specific medical outcomes

Medical results vary by individual. No guarantees are made regarding treatment outcomes, weight loss results, medication response, or health improvements.

All clinical decisions are made in accordance with medical standards and applicable Kansas law.
