  • Follow prescribed treatment plans and medication instructions

MediZen Revitalize L.L.C. does not provide emergency medical services.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

3. Appointments, Payments, and Cancellations

All services require advance payment through approved payment processors, including but not limited to Square and other authorized platforms.

Appointments will not be confirmed until:

  • Payment has been received

  • Required intake forms and consent documents have been completed

Cancellation Policy:

  • Cancellations must be made at least 24 hours in advance

  • No-shows or late cancellations may result in forfeiture of fees

  • Refunds are not guaranteed and are issued at the discretion of MediZen Revitalize L.L.C. in accordance with service policies

Medication costs, pharmacy fees, and third-party service fees are separate and may be non-refundable.

4. Eligibility for Services

Services are available only to individuals:

  • 18 years of age or older

  • Physically located in Kansas at the time of consultation

  • Medically appropriate for virtual treatment as determined by the provider

We reserve the right to decline, discontinue, or terminate services at our clinical discretion.

5. Prescription and Treatment Decisions
​

All treatment recommendations, including prescription medications, are based on provider evaluation and medical appropriateness.

Submission of payment does not guarantee:

  • A specific diagnosis

  • Approval of a requested medication

  • Issuance of a prescription

Clinical decisions are made solely at the discretion of the licensed provider in accordance with applicable laws and medical standards.
